Four easy steps to join our team

Become appointed with Ameritas or Ameritas of New York and comply with HIPAA Privacy regulations by completing the Appointment Application/Business Associate Addendum.

If needed, view the HIPAA Business Associate Addendum on its own.

Mail or fax the application and addendum, along with a copy of your license, to the Group Sales Office nearest you.

You have the option to deposit your commissions directly to your account. Just complete the Direct Deposit Authorization.

Network dentists charge 25-50% less than their regular rates, providing out-of-pocket savings to members. And the Ameritas network includes providers in the U.S. and Mexico.
